DOCUMENTARY REQUIREMENTS:
For New Accreditation
- Authenticated Manning Agreement (MA) by Philippine Consular Office/Embassy
- Authenticated Special Power of Attorney (SPA) by Philippine Consular Office/Embassy
- Appointment Letter of Principal
- Vessel's/Platform's Particulars, Type, Ex-name and IMO number
- Vessel's/Platform's Certificate of Registration
- Vessel's/Platform's Certificate of Cover (P & I Insurance)
- Vessel's/Platform's Crew Complement and Salary Scale Form
- Crew Request of Principal
- Valid Commercial Registration or Business License of the Principal
- Passport Copy of Company's Authorized Signatory
Note: Provisional Accreditation is issued when Manning Agreement and Special Power of Attorney are not verified/authenticated.
For Renewal of Accreditation
- List of Active and Inactive Vessels/Platforms
- Vessel's/Platform's Crew Complement and Salary Scale Form, if Amended
- Renewed Manning Agreement and Special Power of Attorney
- Confirmation from Agency and Principal Re-adoption of Same Salary Scale
For Accreditation of Taiwan Principal Hiring Filipino Fishermen
- Authenticated Manning Agreement (MA) by Philippine Consular Office/Embassy
- Authenticated Special Power of Attorney (SPA) by Philippine Consular Office/Embassy
- Authenticated Council of Labor Affairs (CLA) Approval
- Valid Fishing License
- Valid Fishing License for Overseas Operation, if Applicable
- Certificate of Vessel Registry (CVR) or any Equivalent Official Document of the Ship
- Crew Request of Principal
- Vessel's Crew Complement and Salary Scale Form
